Medical and dental build outs sit in their own category. They look like office work from the outside, but the code, MEP, and finish requirements are closer to a small hospital. Here is what physicians, dentists, and healthcare investors should know before starting a project in Georgia.

    Typical cost ranges

In Canton, Woodstock, Alpharetta, and across North Metro Atlanta:

    • General practice dental, 2,000 to 3,500 sq ft: $300 to $475 per sq ft
    • Specialty dental (oral surgery, orthodontics): $350 to $525 per sq ft
    • Primary care medical office: $250 to $400 per sq ft
    • Specialty medical (imaging, infusion, surgical): $400 to $700 per sq ft

These ranges assume a build out into an existing shell. Ground up adds significant cost.

What drives medical and dental pricing up

    Plumbing

Multiple operatories or exam rooms each need water, drain, vacuum, and often medical gas. Plumbing scope can be 3 to 5 times what an equivalent office would need.

    Mechanical

Code driven ventilation rates, dedicated exhaust, and zoning for waiting versus treatment areas. Some specialties require negative pressure rooms.

    Electrical

Higher load density, dedicated panels, emergency power for select equipment, and code required lighting in treatment rooms.

    Lead and shielding

Imaging, certain dental, and some specialty rooms require lead lined walls and protected doors. This is specialty work that needs to be planned early.

    Finishes

Cleanable, durable surfaces. Sealed flooring with welded seams in many treatment areas. Solid surface countertops. Antimicrobial wall protection.

    Permitting and code

In Georgia, medical and dental build outs typically trigger:

    • City or county building permit
    • Plumbing, mechanical, and electrical sub permits
    • State Department of Community Health review for certain uses
    • Department of Public Health where applicable
    • ADA compliance review

Plan for 6 to 12 weeks of permitting depending on jurisdiction and use type.

    Timeline expectations

For a typical 3,000 sq ft dental or primary care build out in Cherokee County:

    • Design and permitting: 8 to 14 weeks
    • Construction: 12 to 20 weeks
    • Equipment installation and inspections: 2 to 4 weeks
    • Total from signed contract to opening: 6 to 10 months

Specialty practices and imaging centers run longer.

    Common mistakes

    1. Choosing space without checking MEP capacity in the existing building
    2. Underestimating ceiling height required for HVAC and lighting
    3. Ordering equipment before construction documents are coordinated
    4. Skipping a real ADA review until late in the project
    5. Using a contractor with no healthcare experience
